AFFORDABLE CARE ACT

Objective: Led a multidisciplinary team to implement the Affordable Care Act with Medi-Cal expansion within six health centers. Planned and hosted two Covered California outreach and enrollment health fairs

Deliverables: Transitioned 11,000 patients to new health plans, developed processes, outreach and marketing collateral to communicate changes to patients, developed workflow algorithms and scripts for staff, trained staff, implemented continuous quality improvement.

Outcomes: New processes and change management plans were developed; staff were been trained; patient communication was developed; 11,000 patients were transitioned. As the project lead, I directed all aspects of the project from planning to quality improvement.


DEVELOPMENT OF HEALTH EDUCATION PROGRAM

Objectives: Developed a new evidence-based health education program, coalesced multiple independent programs into an umbrella program, including childhood obesity, asthma, smoking cessation, diabetes, and cardiovascular fitness and nutrition. Developed new curriculum, educational materials and internal processes, developed encounter notes documentation with embedded clinical outcomes for evaluation and quality improvement, recruited and trained staff

Deliverables: Health education programs portfolio development and maintenance for three large health center sites, developed or collaborate with partners to conduct health education classes or workshops, including but not limited to smoking cessation, gun safety, nutrition and physical activity, asthma, diabetes wellness and other topics. Developed memorandum of agreement for partnerships

Outcomes: Health education classes and events were initiated, studied and operationalized, all previously independent programs were consolidated, all current curricula were analyzed and updated, internal processes were analyzed and revised for efficiency, staff were trained, and competency was assessed.

Long term outcomes: Improved knowledge of chronic disease and healthy living for participants, improved behaviors in food and exercise choices, improved quality of life and health for participants


INSTRUCTIONAL MANUAL DEVELOPMENT, ELECTRONIC HEALTH RECORD

Objectives: Developed instructional manual (job-aide) for pediatric asthma team on all aspects of scheduling, referrals processing, encounter note documentation, patient recalls, and telephone follow up processes for EPIC electronic health record

Deliverables: Annotated instructional manual was developed, processes have been documented with step-by-step annotated instructions for practice management and encounter documentation for staff providing asthma patient education, home environmental assessments, and telephonic follow-up and case management, continuous improvement of instructions is ongoing

Outcomes: Staff are well versed in efficient encounter documentation in electronic health record; new hires have a reference and training manual to follow


SPIROMETRY TESTING

Objectives: Implemented spirometry (pulmonary function) testing at four health center sites for physicians and support staff                                     

Deliverables: Implemented training program for project team, conduct 10-15 tests per month per health center site; developed processes for patient scheduling, registration, testing, electronic health record documentation, and quality improvement. Trained pediatricians and support staff, tested patients, and operationalized processes

Outcomes: Improved diagnosing and assessment of patients with asthma, improved management of asthma control


TRAINING AND COMPETENCY ASSESSMENT PROGRAM, PEDIATRIC ASTHMA DISEASE MANAGEMENT PROGRAM

Objectives: Developed multi-module bilingual training program, including curricula, schedule and competency assessments for the provision of patient health education, case management, and home environmental assessment visits. Continuous improvement of curricula and competencies

Deliverables: Developed comprehensive bilingual curricula, train, coach, and assess the competency of staff with developed training program

Outcomes: Multi-module curricula developed, staff trained, competency assessed and staff deployed


HEMOGLOBIN A1c POINT OF CARE TESTING, PRIMARY CARE

Objectives:  Conducted point of care, walk-in, Hemoglobin A1c testing of 300 patients with diabetes or at risk for diabetes, four times per year, every three months

Deliverables: English and Spanish outreach and communication plans developed; My Diabetes Passport educational and tracking tool created; outreach postcards sent, workflow and training materials, standing order, patient survey and electronic health record documentation tools developed
